Inspector Notes

A renewal inspection was conducted on 4/28/2022. 2 children (4pts) were in attendance with the Provider. The physical space, 2 children's records, Provider/household member documents, posted documents, evacuation drills, attendance records and other required documents were reviewed. No medications were on site. Children were observed interacting with the Provider, eating morning snack and engaged in morning activities. Age appropriate materials were available for the children. The information gathered during the inspection determined areas of non-compliance with applicable standards or law and violations were noted on the Violation Notice given to the Provider. Based on record review, the Provider did not obtain repeat Central Registry Searches as required. Evidence: The Central Registry Search results for the provider and household members were expired.

Based on record review and interview with the Provider, the Provider did not obtain subsequent tuberculosis screenings for caregiver and household members. Evidence: The current TB screenings for the caregiver and household members are expired.
