During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated on 4/9/2025 and completed on 4/9/2025, as a part of the licensure period. The on-site inspection began at 9:18am and ended at 10:40am. There was one child present and one caregiver; the point total was two. The inspector reviewed two children?s records and three caregiver/household members records on-site. This inspection included document review and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
6The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
There was no physical exam record available for Child #2, who had been in care for two years and two months.
Staff A's CPR certification was outdated by fifteen days.
Staff A's first aid certification was outdated by fifteen days.
Areas and furnishings of the family day home, inside and outside, must be maintained in a clean, safe, and operable condition.
The children's wooden diaper changing table had six areas of chipped and peeling paint.
Pet A and Pet B's rabies vaccinations were outdated by four months.
When meals or snacks are provided by the family day home, the menu for the current one-week period must be dated and given to parents, posted, or placed in an area accessible to parents.
The menu posted for parents was outdated by fifteen weeks.
