Inspector Notes

A Subsidy Health and Safety inspection was conducted on November 10, 2022 to review supplemental health and safety requirements. This on-site visit was conducted from 3:30 p.m. through 4:50 p.m. Upon arrival, there were 45 children and 5 staff supervising. Five children records and five staff records were reviewed and updated.

A licensing code compliance inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Areas of non-compliance may be found on the violation notice. Please complete your plan of correction within 5 business days and return to the licensing inspector.

Based on record review the center failed to have one staff member complete 16 hours of annual training to include the departments health and safety course update.
Evidence:
1. Five staff records were reviewed and updated.
2. Staff #2 and Staff #5 had not completed the departments 3 hour health and safety update.
3. Staff #2 completed the required 10 hours of health and safety training on 12/22/2020.
4. Staff #5 completed the required 10 hours of health and safety training on 11/29/2020.
5. The director confirmed the 3 hour update had not be completed for Staff #3 and Staff #5.
Plan of Correction: Staff #2 and staff #5 have started the DSS Virginia Health & Safety update training and will complete the training by December 1st. We have documented the required update training for staff?s annual review and completion.
