Inspector Notes
An unannounced monitoring inspection was conducted on October 23, 2023, from 3:30 p.m. through 4:40 p.m. Upon arrival, there were 45 children, ages 5 through 10, in the care of 6 staff.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records were reviewed. Annual review of staff records will be scheduled later.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations
2Evidence:
1. The medication and medication paperwork was reviewed for two children.
2. Child #1 and Child #2 did not have documentation of parent authorization to administer prescription medicine.
3. Child #1 no authorization for Epi Pen.
4. Child #2 no authorization for Albuterol.
5. Staff confirmed medicine authorization could not be found.
Evidence:
1. The medication and medication paperwork was reviewed for two children.
2. Albuterol for Child #2 was did not have the pharmacy label attached to include the name, dosage amount and times to be given.
3. Staff confirmed the Albuterol did not have a pharmacy label attached.
