Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on January 13, 2022. There were 21 children present and in the care of 2 staff. The inspector reviewed compliance in the areas of administration, physical plant, staff qualifications, staffing and supervision, physical plant, programming, special services, special care provisions and emergencies. A total of 3 children's records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or law and the violations were documented on the violation notice issued to the program.

Based on review of staff records and verification with staff, the center failed to obtain a completed criminal history record check prior to employment.

Evidence:
1. The record for Staff Member #1 contained a completed criminal history record check dated 09/14/2021. Date of hire for Staff Member #1 was 09/11/2021.
2. Staff Member #1 confirmed the completed criminal history check was received after the date of hire.
Plan of Correction: The staff is aware this search needs to be completed before the date of hire and moving forward will ensure it is.

Based on review of staff records and discussion with staff, the center failed to obtain a completed sworn statement prior to employment.

Evidence:
1. The record for Staff Member #2 contained a completed sworn statement dated 11/30/2020. The form which was completed was not the current Sworn Statement and did not contain one of the five required questions. Date of hire for Staff Member #2 was 11/30/2020.
2. Staff Member #1 confirmed that completed form was an old version.
Plan of Correction: The most current version of the Sworn Statement will be used from now on.
