Inspector Notes
A Subsidy Health and Safety inspection was conducted on November 29, 2022 to review supplemental health and safety requirements. This onsite visit was conducted from 3:30 p.m. through 5:00 p.m. Upon arrival, there were 21 children 3 staff supervising. Four children records and five staff records were reviewed and updated.
A licensing code compliance inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Areas of
non-compliance may be found on the violation notice.

Violations
1annual training to include the departments health and safety course update.
Evidence:
1. Five staff records were reviewed and updated.
2. Staff #4 and Staff #5 had not completed the departments annual 3 hour health and safety update.
3. Staff #4's completed the required 10 hours of health and safety training on 01/21/2021.
4. Staff #5 completed the require 10 hours of health and safety training on 06/05/2019.
5. The director confirmed the 3 hour update had not be completed for Staff #4 and Staff #5.
