Inspector Notes

Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 29, 2022, from 3:30 p.m. through 5:00 p.m. Upon arrival, there were 21 children, ages 5 through 10, in the care of 3 staff. Four children records and five staff records were reviewed and updated.

Areas observed were homework power hour, hand washing bathroom procedures, transition and free play in the gymnasium. The children seemed to be well aware of the daily routine and schedule.

Based on record review the center failed to have one staff member obtain an updated tuberculosis screening.

Evidence:

1. Five staff records were reviewed.
2. Staff #5's last documented tuberculosis screening was dated 05/2020.
3. The director confirmed Staff #5 did not have documentation of obtaining an updated tuberculosis screening.
