Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated on 2/16/2022 and concluded on 2/17/2022. The director was contacted by telephone to initiate the inspection. There were 37 pre-school and school age children present and three staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and three staff records, along with any requested program records submitted by the facility to determine if required documentation was complete
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program..

The most recent background checks for staff #3, who was on duty at the time of the inspection, were all over five years old. The dates are noted below:
Sworn statement or affirmation - 9/29/16
Fingerprints - 10/10/16
Search of the central registry - 10/14/16
All staff records will be reviewed and any expired background checks will be updated.
Evidence:
1. The last Tb screening for staff #1, who was on duty at the time of the inspection, was dated 11/20/19 and was, therefore, over two years old.
2. The last Tb screening for staff #2, who was on duty at the time of the inspection, was dated 11/18/19 and was, therefore, over two years old.
All staff records will be reviewed to ensure that all Tb screenings are current.
Evidence:
1. There was a child enrolled who had an Epi-pen, Benedryl and Albuteral on site.
2. There was not a staff member currently assigned to the site who has current MAT training.
