During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes

An unannounced ,on-site monitoring inspection was initiated on 03/06/2025 and completed on 03/06/2025. The on-site inspection began at 3:08 pm and ended at 4:30 pm. The inspector reviewed compliance in the areas listed above. There were 38 children present and 3 staff. A 4th staff arrived at approximately 3:45 pm. The inspector reviewed 5 children's records on-site and 2 staff records electronically on 03/10/2025.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The inspection documents from the most recent licensing inspection were not posted.
Plan of Correction: The License and all other documents will be posted on the premises.

All employees shall undergo a background check in accordance with subsection B prior to employment and every five years thereafter.

The most recent criminal record check for staff 1 was completed more than five years ago.

When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.

38 children, ages 4 years - 9 years, were observed with 2 staff during toileting and hand washing in the hallway area of the school. 4 staff were needed to meet the required ratio for this mixed age group.
Plan of Correction: The staff-to-children ratio and group size will be applicable to the youngest child in the group and will apply to the entire group.

The center shall maintain a record of the dates of the practice drills for one year.

The February 2025 fire drill was not listed on the emergency fire drill log for the program.
Plan of Correction: The center will maintain a completed record of all drills for one year.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following:
