A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/06/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-790-600-D-1
All staff who work directly with children shall have within 90 days of the date of employment current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training shall include an in-person competency demonstration.
There was not documentation that staff 3 has completed CPR certification. Staff 3 has been employed for more than 90 days and was working on the date of the inspection.
Plan of Correction: Program Director has enrolled staff in a CPR course. Certificate will be documented in staff file. Program Director will ensure that all staff CPR will be completed prior to employment date. Program Director has created a staff audit spreadsheet to ensure compliance of staff files.

All staff who work directly with children shall have within 90 days of the date of employment current certification in first aid appropriate to the ages of children in care.
There was not documentation that staff 3 has completed first aid certification. Staff 3 has been employed for more than 90 days and was working on the date of the inspection.

Staff who work directly with children shall annually attend the department's health and safety update course.
There was no documentation that staff 1 and staff 2 have completed the department's 3-hour annual health and safety update course. Both staff have been employed for more 3 years.
Plan of Correction: Program Director will ensure that all staff Virginia Department of Education sponsored orientation is completed within 90 days of employment date. Program Director has created a staff audit spreadsheet to ensure compliance of staff files.
