Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated on 3/17/2022 and concluded on 3/29/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 23 children present, ranging in ages from 2 years to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
Based on review of two staff files and staff interview, the facility did not have a search of the central registry check (CPS check) within 30 days of hire. Staff 2 (date of hire 2/21/2022) did not have a search of the central registry on file. According to staff, the search of the central registry was not sent within the first 7 days as required. The check was not returned by 3/29/2022 according to staff (HR).
Plan of Correction: Staff 2 had an issue with getting the form notarized and it was not mailed until 3/10/2022. All CPS checks will be sent within 7 days of hire.
Standard 8VAC20-780-160-A-1
Based on review of two new staff files and staff interview, the facility failed to have documentation of a completed TB screening at employment and prior to direct contact with children. Evidence: Staff 1 (date of hire 3/7/2022) did not have a TB screening or test.
Plan of Correction: Staff 1 was scheduled to get a TB test or screening 3/17/2022.
