Inspector Notes

An unannounced renewal inspection was conducted on 01/03/2023. An additional electronic record review was conducted on 01/06/2023. There were 65 children, ages 4 months- 6 years, and 15 staff members providing direct supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed during outdoor time, mealtime, and rest time. The inspector discussed the following with the provider: emergency supplies and children?s records. A total of 5 children?s records, 5 staff records, and 1 agent record were reviewed. The inspector arrived for the inspection at 10:30 am and departed at 1:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Based on record review, the center failed to ensure that every applicant for licensure as a child day center shall undergo a background check in accordance with the Code of Virginia prior to issuance of a license and every five years thereafter.
Evidence: The most recent search of the central registry on record for Agent 1 was dated 07/25/2017. There was not a repeat search of the central registry obtained in 2022.
Plan of Correction: Provider will request a search as soon as possible.
