Inspector Notes

An unannounced inspection was initiated on 4/22/2025 and completed on 5/2/25 in response to a complaint received by the licensing office on 4/16/2025 relating to allegations of ratio. The inspector did not review any records. This inspection included tour of the facility, and interviews.

After review with the LA, the inspection document was updated on 6/5/2025.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/9/2025. A POC submitted after this date will not appear on the public website.

The required ratio for children birth to 16 month is one staff per four children.

On 4/22/2025, one staff member was observed supervising eight children (youngest child 12 months old) for approximately 6 minutes.
Plan of Correction: Adjust staff schedule effective immediately. Also adjust drop off times as of June 1.

When children are in ongoing mixed age groups, the staff to child ratio and group size applicable to the youngest child in the group shall apply to the entire group.

On 4/22/2025 during morning drop off, Staff 1 was observed supervising up to 19 children, ages 18 months to two and a half years old. The required staff ratio for this group of children was 1:5 and maximum group size was 15. Four staff would be required.
Plan of Correction: Adjust staff schedule effective immediately. Also adjust drop off times as of June 1.
