Inspector Notes

An unannounced inspection was initiated on 4/7/2026 and completed on 4/7/2026 in response to a complaint received by the licensing office on 2/18/2026 relating to allegations of administration, staffing and supervision and forbidden actions. The Department issued a Special Order on 1/28/2026 to place the licensee on probation for a period 3 months and notify parents as a result of high risk violations. This inspection included interviews and tour of the facility.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
