Inspector Notes

An unannounced monitoring inspection was conducted today. The inspection focused on a self report regarding behavioral guidance and a forbidden action. Staff interviews were conducted, observation of classrooms was made, staff and child files were reviewed and video footage recorded on 02/15/2024 was viewed. The preponderance of evidence supports a valid finding regarding a forbidden action. A mandated monitoring inspection was also conducted today. All areas of the standards were reviewed. Standard 8VAC20-780-410-1
Based on observation of the center's own video recording and staff interviews, a forbidden action was used with one child.
Evidence:
1. Child #1 (3 years) was jumping on a cot during naptime on 02/15/2024,
2. Staff #1 admitted to placing their thumb and index finger on Child #1 's face, (3 years old) because Child #1 wasn't listening to them.
3. The center's video footage on 02/15/2024 showed Child #1 jumping repeatedly on their cot when Staff #1 walked over to Child #1 and roughly handled Child #1 by forcefully grabbing Child #1's chin with their right hand.
Plan of Correction: Provide all staff training during professional development day on 2/19/24 including behavioral guidance, support, classroom management, technique and mandated reporter training reminder. Participate in weekly classroom observations providing teacher and student guidance and feedback for 30 days Ongoing. Admin will continue to provide classroom support as needed.
