Inspector Notes

An unannounced monitoring inspection was conducted today between 12:00pm and 12:45pm. There were 4 children present with the Provider and for a total of 8 points within the supervision guidelines. The children were observed playing with building blocks and reading flash cards. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. There was no medication observed on-site and the provider is not currently trained to administer medication. Violations were found during the inspection and can be found on the Violation Notice.

Standard 8VAC20-800-180-A
Based on record review and interview, the provider has not obtained a subsequent tuberculosis (TB) screening for each caregiver every two years.
Evidence:
1. Caregiver #1's most recent TB screening is dated 2/7/2020.
2. Caregiver #2's most recent TB screening is dated 2/7/2020.
Plan of Correction: I will make appointments for us to go next week.
Standard 8VAC20-800-210-A
Based on interview, each caregiver has not obtained a minimum of 16 hours of training annually in areas relevant to their job responsibilities.
Evidence: Caregiver #2 did not complete any job related training in 2021. This was previously cited for the same caregiver in 2020.
Plan of Correction: I will have them start completing the trainings with me.
