Inspector Notes
An unannounced renewal inspection was conducted today between 1:45pm and 2:30pm. There were 3 children present with the Provider for a total of 6 points within the supervision guidelines. The children were observed playing freely and resting. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
4Evidence: The children's bathroom did not contain paper towels.
Evidence: The Supplemental form, containing information from children's records, from the most recent inspection was posted on the wall.
Evidence: The providers first aid kit did not contain gauze pads, tape and disposable gloves.
Evidence: The provider attempted to turn on their battery-operated radio and it did not work. The provider stated that they did not have a working flashlight.
