Inspector Notes
An unannounced renewal inspection was conducted today between 11:00am and 12:05pm. There were 4 children present with the Provider for a total of 9 points within the supervision guidelines. The children were observed playing freely during the inspection. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
5Evidence: A household member turned 14 years old in June 2023. The provider confirmed that central registry background check results have not been received.
Evidence: The child care program takes place in the providers lower level and stairs lead to the basement program. A child safety gate was observed leaning against the wall at the bottom of the staircase. Child #4 is 17 months old and can access the staircase.
Evidence: The supplemental page of the most recent inspection was observed posted on the providers wall. The supplemental page clearly states "DO NOT POST".
Evidence:
1. Child #2's record did not contain addresses for both emergency contacts.
2. Child #3's record did not contain the families medical insurance policy number.
3. Child #3's record did not contain the Information for Parents document.
Based on observation, meals and snacks brough from home was not clearly labeled to identify the owner.
Evidence: Four out of four lunch totes were observed were not labeled with each child's name.
