Inspector Notes
An unannounced, on-site renewal inspection was initiated on July 15, 2024 and completed on July 15, 2024. The inspection began at 9:10am and ended at 10:30am. There were four children present and one staff person. The inspector reviewed two children's records and one staff and one household member. Areas of non-compliance are identified on the violation notice.Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
