During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 5/7/2025. The on-site inspection began at 12:50pm and ended at 2:50pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 39. The inspector reviewed 5 children?s records and 4 caregiver/household members records on-site on 5/7/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/19/2025. A POC submitted after this date will not appear on the public website.
Violations
7Household member #2 turned 14 years old more than 30 days ago and did not have documentation of the results of a central registry search on file at the time of the inspection.
Upon arrival to the family day home, the Inspectors counted 8 children in the daycare space in the basement of the home. The Provider stated that there were only 8 children, but did not disclose to the licensing inspectors that there were 3 additional children in an upstairs
room of the home. As the Inspectors toured the basement daycare space, they could hear
children upstairs exiting the front door of the home. Upon observation, Household Member
#1 was observed walking down the sidewalk towards the playground with Child 1, Child 2,
and Child 3, all of whom did not have any shoes on. It was confirmed by HHM #1 and the Provider that the Provider had asked the HH Member to take the 3 children to the playground since the inspectors were going to tour the entire home. The Provider wasdishonest regarding the additional 3 children in care which put her over her capacity and her points. The HH Member was asked by the inspectors to return to the home with the children prior to reaching the playground, which they did. Once back in the family day home, the Provider had a neighbor (male) come to pick up Child #1. The inspectors asked for the neighbor's ID to verify that the neighbor was a parent or guardian listed on Child #1's child record. The neighbor stated that they did not have an ID with him and left. the home. The inspectors did not allow the neighbor to pick up Child #1 due to lack of identification and written authorization from the parents.The Provider stated that the neighbor was a guardian and was authorized to pick up the child, however, this was not true.
At the time of the inspection, the Provider was over the capacity allowed by the terms of the license. The capacity listed on the Provider's license is 10 children and the Provider was found to have 11 children in care.
The Provider was observed to have 11 children in care at the time of the inspection when the Provider's maximum licensed capacity is 10 children.
Child #1, Child #2, and Child #3 were all napping in a large bed located in an upstairs bedroom at the start of the inspection. Each child was not provided with an individual rest mat or bed for napping.
The Provider was observed to have 11 children (39 points) with 1 assistant caregiver at the time of the inspection. The Provider exceeded the 16 points alotted per caregiver.
Child #1, Child #2, and Child #3 were observed to be taken off of the premises of the family day home to head towards the neighborhood playground by Household member #1 without having documentation of a general written permission form for regularly scheduled trips on file.
