Inspector Notes

An unannounced, on-site non-mandated monitoring inspection was initiated and completed on 6/12/2025. The on-site inspection began at 12:40pm and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 21The inspector reviewed 0 children?s records and 1 caregiver/household member records on-site on 6/12/2025.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-120-A
The provider is required to maintain a complete record for each caregiver including documentation that two or more references as to character and reputation as
well as competency were checked before employment.

Caregiver #1 did not have documentation of at least two references being checked on file prior to employment.
Plan of Correction: I will make sure to check references before hiring caregivers and I will get the missing reference for this caregiver.
