Inspector Notes
An unannounced monitoring inspection was conducted today between 9:05am and 10am. There were 9 children present with two assistant caregivers for a total of 27 points within the supervision guidelines. The provider was not home at the start of the inspection. The children were observed getting dropped off, playing freely, being read to and playing outside. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

2Evidence: The Supplemental form from the most recent inspection was observed posted at the family day home. The Supplemental form clearly stated "DO NOT POST" and noted that the information should be kept confidential.
Evidence: Two occupied cribs in the nap room were located approximately 5 inches apart.
