During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 7/29/2025 from 12:00pm - 1:15pm. There were 11 children and 3 caregivers present during the time of the inspection; totaling 30 points. The inspector reviewed 4 child records and 4 caregiver/household records onsite. The inspection included document review, interviews, observations, and a walk thru of the program.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which is 8/12/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-90-A
REPEAT VIOLATION
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to requirements of the Code of Virginia and applicable State Board of Health regulations.

Documentation for Child #1 was obtained 3 weeks after the first date of attendance.
Documentation for Child #2 was obtained 2 days after the first date of attendance.
Plan of Correction: The provider will ensure immunization records for new children are received by the first day.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations once every 6 months for children under the age of 2 years.

Child #3, enrolled for 1 year, did not have documentation of immunizations every 6 months while under the age of 2.
Plan of Correction: The provider will ensure updated immunizations are received as required.
Standard 8VAC20-800-900-B
Children shall not be allowed to eat or drink while walking, running, playing, lying down, or riding in vehicles.

Child #1, age 11 months, was observed in a crib with a bottle.
Plan of Correction: Caregivers will not give bottles to children in the cribs.
