Inspector Notes

An unannounced monitoring inspection was conducted on July 18, 2023 between the hours of 7:50am and 9:10am. There were a total of 34 children with 8 staff supervising. Children were observed arriving for the day, diapering, and engaged in age appropriate activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on review of children?s records, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #1 (DOB 09/2021) did not have an updated immunization record. The last documentation is 9/22/21.
Plan of Correction: I will speak to the parents today.

Based on observation, staff did not wash their hands with soap and running water before feeding children.
Evidence: Staff #5 did not wash their hands before feeding Child #5.
Plan of Correction: I will speak to my staff today.

Based on review of children?s records, the center failed to maintain records with documentation of the parent?s work phone number and proof of viewing the child?s identity and age.
Evidence:
1. Record for Child #2 did not have documentation of the parent?s work phone number.
2. Record for Child #4 did not have documentation of staff viewing the child?s identity and age.
Plan of Correction: I will get the parents to provide the telephone numbers this week.
