During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on March 17, 2025. The on-site inspection began at 10:20am and ended at 1:05pm. The inspector reviewed compliance in the areas listed above. There were 54 children present and 14 staff. The inspector reviewed eight children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years. The most recent sworn statement for staff #5 was overdue by a year and eleven months.
Plan of Correction: This will be correct immediately. We are in the process of reviewing all our files to ensure all required documents are current.

Violation was corrected during the inspection.
Standard 8VAC20-780-520-A
Over-the counter skin products shall not be kept or used beyond the expiration date of the product.

Three diaper creams were kept beyond the expiration date of the product. Classroom #1 had two diaper creams expired. Child # 6's diaper ointment expired on two months ago. Child #7's diaper ointment expired 6 months ago. Classroom #2 had one diaper cream expired. Child #8's diaper ointment expired three months ago.
Plan of Correction: We will review the policy with all staff to check the expiration dates on the diaper ointment. Expired ointments will be sent home or thrown away today.
