Inspector Notes

An announced monitoring inspection was initiated on 10/17/2025 and completed on 10/17/2025 with Loudoun CPS in response to a self-report received by the licensing office on 10/8/2025 relating to forbidden actions and behavioral guidance. The inspector reviewed 1 child record and 2 staff records on-site on 10/17/2025. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts

A video was reviewed for an incident that occurred on October 8,2025, showing staff member #1 approaching child #1 (age: 18months) after child #1 took another child's water bottle. Staff member #1 got down on the child's level, grabbed child #1 on both upper arms and in loud voice said, "No throwing." Child #1 was visibly upset.
Plan of Correction: Staff member was terminated on 10/8/2025.

A video was reviewed for an incident that occurred on October 8,2025, showing staff #1 grabbing child #1 (age 18 months) by both upper arms causing their body to wobble. Child #1 was visibly upset.
