Inspector Notes

A monitoring inspection was conducted on 11/01/2023 between the hours of approximately 9:50 am and 11:10 am. There were 33 children present, ranging in ages from 3 to 5 years, with 6 staff supervising. Children were observed while on learning activities, circle time, tracing their names and playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on documentation review, the written list of all children's allergies, sensitivities, and dietary restrictions did not meet all requirements.
Evidence:
1) The allergy lists available for review were not dated as required.

Based on record review, parent confirmation that the required information in the child's record is up to date and documentation of the review, was not requested at least annually.
Evidence:
1) The most recent information review in the record for Child #3 was dated 05/24/2022 and for Child #4 was dated 04/04/2022.

Based on record review, one child record did not contain all required information.
Evidence:
1) The record for Child #3 did not contain the name, address, and phone number of one designated person to call in an emergency if a parent cannot be reached.
