Inspector Notes
A monitoring inspection was conducted on 02/07/2024 between the hours of approximately 10:00 am and 11:10 am. There were 29 children present, ranging in ages from 3 years to 5 years, with 6 staff supervising. Children were observed while working in different learning stations, working with manipulatives, and solving puzzles.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 1 staff record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 1) On the date of inspection, the findings of the most recent inspection of the facility (11/2023) were not posted on the green room.
Evidence:
1) On the date of inspection, the most recent inspection report available for review was dated 04/19/2022.
Based on documentation review, the written list of all children's allergies, sensitivities, and
dietary restrictions did not meet all requirements.
Evidence:
1) The allergy lists available for review were not dated as required.
Based on record review, the center did not request at least annually parent confirmation that the required information in the child's record is up to date.
Evidence:
1) The most recent review of the emergency information documented in the record for Child #2 was dated 01/13/2023.
Evidence:
1) The record for Child #1 did not contain the address of a second designated person to call in an emergency if a parent cannot be reached.
2) The record for Child #2 did not contain the first date of attendance.
