During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on March 25, 2026. The on-site inspection began at 2:35 pm and ended at 3:30 pm. The inspector reviewed compliance in the areas listed above. There were six children present and one caregiver; the point total was thirteen. The inspector reviewed five children?s records and one caregiver record on-site.

This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children's records are required to contain insurance plan/number and signed acknowledgement from parent for receipt of required information. The records for Child #1, #3 and #4 did not have parent's signed acknowledgement of whether provider has liability insurance coverage. The record for Child #3 did not have insurance plan information.
Plan of Correction: Information will be obtained from parents today.
