Inspector Notes
An investigation was initiated from 2:45 pm - 3:30 pm to investigate a complaint regarding diapering. Interviews were conducted and documentation was reviewed. The investigation regarding diapering is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interviews it was determined that not all procedures for diapering were followed to include that children shall be cleaned and changed immediately when their diapers or clothes become wet or soiled. Evidence: 1.) On 05/09/2023 Staff A was assisting Child A (Age 2 years) with toilet training while diapering. Staff A instructed child A to throw their diaper in the trash. The diaper was not disposed of and the wet diaper was still attached around the child's leg and a new diaper was placed on child A. Upon picking up child A from school it was discovered that child A had on two wet diapers. 2.) On 06/09/2023 around 4:00 pm child A was picked up from school with a soiled diaper. Upon review of child A's daily activity report it was discovered that the last diaper change recorded for child A was at 12:45 pm. It was reported by staff B that child A had a diaper change around 3:00 pm, however the diaper change was not documented.
