During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on 9/30/25 in response to a self-report received by the licensing office on 9/11/25 relating to diaper changing. The inspector reviewed one staff record on-site. This inspection included document review and interview. Information gathered during the inspection determined non-compliance with an applicable standard, and a violation is documented on the violation notice issued to the program.

Submit your POC within five (5) business days from today, which will be the close of business on 10/9/25. A POC submitted after this date will not appear on the public website.

When a child?s clothing or diaper becomes wet or soiled, the child is required to be cleaned and changed immediately.

On 9/10/25, Child A, 3-years-old, arrived at the center from a public-school bus at approximately 1:00pm. Staff #1 reported checking Child A?s diaper at that time and that the diaper was wet, but not enough to warrant changing it. Staff #1 reported checking Child A?s diaper again at approximately 4:00pm and said it was wet, but did not contain feces, so again did not change the child. When Child A?s parent arrived at the center to pick up the child shortly after 4:30pm, they reportedly noted Child A was walking unusually and later discovered that the child had a diaper rash.
Plan of Correction: The staff member involved in the incident, Staff #1, was terminated on 9/11/2025. Immediately thereafter, a full review of diaper changing policies and procedures was conducted with all staff to ensure understanding and compliance with respective licensing standards. All staff completed a refresher hands-on training on proper diapering practices. Ongoing monitoring of these practices by directors is taking place to ensure required diapering procedures and protocols are consistently followed by all staff.
