Inspector Notes
An unannounced monitoring inspection was conducted on May 9, 2024 from approximately 10:15 am to 1:30 pm. There were 76 children in attendance and a total of 18 staff present. 10 staff records and 10 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and playgrounds. The children were observed playing outside, playing on the fun bus, getting ready for lunch, and taking a nap, while the licensing inspector was on-site.
The first aid kit was inspected and found complete. Evacuation drills were conducted monthly with the most recent conducted on 04/29/24.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 05/14/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

2Evidence: The toddler playground contained a tree with several wooden stakes tied to it. The wooden stakes were approximately 2 feet in height and were staked into the ground and pointing out diagonally. The tree was located on a small hill on the toddler playground accessible to the toddler children. The stakes had rough, pointed edges that could snag skin.
Evidence: The refrigerator in classroom 1 contained a container of pureed carrots. The container was not dated or labeled in a way that identified the owner. Interview with staff confirmed that the container was not dated or labeled in a way that identified the owner.
