During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 10/08/2025 and completed on 10/08/2025 in response to a complaint received by the licensing office on 09/22/2025 relating to allegations of staff to child ratio requirements. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/05/2025. A POC submitted after this date will not appear on the public website.

The staff-to-children ratio for children birth up to 16 months old is 1 staff for every 4 children.

During the week of August 18th 2025 there was only 1 staff member present in the classroom during naptime with 8 children. The youngest child in the room at that time was 15 months old.
Plan of Correction: "This situation occurred the first week of the new school year. We had adjusted the age group of this classroom slightly, making the children in this room just under 16 months of age. Staffing schedules were immediately reviewed and adjusted, on August 25th, 2025, to ensure compliance with state-mandated ratios at all times, including during nap periods. Additional staff coverage was assigned to the classroom during rest time to maintain the required ratio. All staff members were reminded of the ratio requirements for each age group and the importance of maintaining compliance throughout the day, including during rest time."
