Inspector Notes
An unannounced renewal inspection was conducted February 10, 2023 from 7:30am to 8:30am. There were 9 (school-agers) directly supervised by 3 staff. The physical plant, 2 staff records, 5 children?s records, 1 child's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in individual and group activities, arriving to care, preparing for and eating breakfast. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
2Evidence: observed that the battery operated radio was not working at the time of the inspection. The staff replaced the batteries, but the radio still would not turn on.
Evidence: reviewed 5 children's records and found that Child 5 did not have documentation of work information for parent 1 and parent 2.
