Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 1/25/22. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children (3 points) present with 1 caregiver supervising. The children were observed engaged in free play. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, special care and emergencies and nutrition. There were no medications on site. Two children's records, one caregiver and one household member's records were reviewed. Emergency plans and drill records were reviewed.

Based on records review, the family day home did not update the Sworn Disclosure Statement (SDS) for each household member, every 5 years.
Evidence: Household Member # 1's SDS was dated 10/28/16, and expired on 10/28/21. The SDS for Household Member # 1 was not updated by that date.
Plan of Correction: The SDS has been updated.

Based on record review, the provider did not review the emergency plan annually.
Evidence: The emergency plan was last updated on 4/18/20. An update for 2021 was not completed.
