Inspector Notes
An unannounced monitoring inspection was conducted today from 11:30 am- 1:30 pm. There were 49 children in care at the time of the inspection directly supervised by 8 staff members. Child to staff ratios were found to be in compliance.
The physical plant, staff records, child records, medication policies and medications, evacuation drills, a sample of injury reports,emergency supplies and policies were reviewed.
Information gathered determined violations with applicable code or law and is noted on the Violation Report.

Violations
3Evidence:
4 cribs in the infant classroom were found to have peeling paint on all top edges.
1 rocking/ bouncy seat was observed to have a bolt sticking out of the attached base.
There is an exposed water drainage tube that is exposed directly in front of the gate used to enter the playground ( tripping hazard).
Mulch needs to be redistributed on the playground, currently observed to be uneven leaving large areas uncovered under swings.
The bouncy seat was removed during the inspection.
The mulch is washing away due to the rain and will be fixed.
Evidence: The sandbox on the playground does not have a cover to protect from the elements and animal. It is also a fall hazard due to the fact that it is not full of sand and has a large drop from the edge to the bottom.
Evidence: The 2's classrom was observed during nap time and 3 cots did not have space of required 12 inches in between for sleeping.
