Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/7/2024. The on-site inspection began at 12:45 pm and ended at 2:30 pm. The inspector reviewed compliance
in the areas listed above. There were 41 children present and 6 caregivers. The inspector reviewed 3 children?s records
and 2 staff records on-site.
This inspection included document review, policies and procedures and emergency planning.
tour of the program, interviews and observation observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-180-B
Based on interview, staff who work directly with children were not capable of communication with emergency personnel.

Evidence:

1. Staff 1 and Staff 2 confirmed they could not communicate with emergency personnel.
Plan of Correction: We will retrain staff about communicating with first responders. We did train during orientation.
Standard 8VAC20-780-280-F
Based on observation substances not stored in the original container were not labeled.

Evidence:
2 spray bottles in the pre-school bathroom were not labeled with contents inside.
Plan of Correction: We will relabel bottles.
Standard 8VAC20-780-440-E
Based on observation there was not at least 12 inches of space between cots and rest mats.

Evidence:
2 Cots in the Toddler 1 and 2 cots in the Toddler 2 room did not have 12 inches of space between them as required.
Plan of Correction: Corrected during inspection.

Based on observation, bottles from home were not labeled.

Evidence:
4 bottles in the infant room did not have date as required.
Plan of Correction: We will remind parents to date and have staff double check.
