During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 2/4/2026 as a part of the licensure period, the provisional licensure period. The on-site inspection began at 12:40 pm and ended at 3:05 pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 6 staff. The inspector reviewed 4 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

The allergy list was not posted in the Pre-K room.
Plan of Correction: We always make sure it's posted. The teacher was cleaning last week and removed it. We will repost today.

Repeat Violation:

There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Exception: Twelve inches of space are not required where cots, beds, or rest mats are located adjacent to a wall or a divider as long as one side is open at all times to allow for passage.

In the 2 year old classroom and Pre- K classroom there were 4 mats that were not 12 inches apart as required.

The center shall notify the parent by the end of the day of any known minor injuries.

The center notification to parents did not have all required information.
