Inspector Notes

A Renewal Inspection was conducted today. There were a total of 5 children(13pts) with the Provider were present when the Inspector arrived. The children were observed sitting in a circle on the floor with the provider engaged in an activity that involved large lego blocks. Observation of the family day home was made. The children were observed interacting with the provider, laughing and playing. The provider assisted the children with washing hands and transitioning to lunch. Lunch was vegetable soup, sliced banana and milk to drink. One infant observed being bottle fed by the provider and placed on their back to sleep. No medication needs identified at this time, one food allergy determined and no topical ointments onsite. All 5 children's records were reviewed, Provider and all household records were also reviewed. Some violations found today. Thank you for your cooperation today. Based on review of children's records, 4 of the 5 records were incomplete.
Evidence:
The record for Child #1,3,4 and 5 did not contain parent's written authorization for emergency medical care.
Plan of Correction: I will have each parent sign this authorization.
Standard 8VAC20-800-90-A
Based on review of child records, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
Child #5 began care on 09/22/2022. Immunization documentation and a statement that this child was adequately immunized has not been obtained.
Plan of Correction: I will call the agency who referred this child and request the immunizations.
