Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on March 31, 2021 and concluded on March 31, 2021. The provider was contacted by telephone to initiate the inspection. There were four children present and two caregivers. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility .
Violations
1Evidence:
1. Caregiver #1 completed the 10 hour preservice on 01/22/2017. Anyone who completed the the pre service training prior to 03/01/2019 had until 03/01/2020 to complete the health and safety updated training. Caregiver #1 completed the health and safety updated training on 05/12/2020 exceeding the required date of 03/02/2020.
2. Caregiver #2 completed the 10 hour preservice on 05/30/2017. Anyone who completed the the pre service training prior to 03/01/2019 had until 03/01/2020 to complete the health and safety updated training and then every 12 months thereafter. Caregiver #2 completed the health and safety updated training on 04/15/2019 however did not renew the training 12 months later.
