Violations

1
Standard 8VAC20-800-70-B
Based on review of two children's records, the provider did not obtain the parent's written acknowledgement of the receipt of the information for parents.

Evidence:

1. The record of child #1, enrolled 02/10/2022, did not contain the parents written acknowledgement of receiving the parent information.
2. The record of child #2, enrolled 01/04/2022, did not contain the parents written acknowledgement of receiving the policies for the administration of medication.
Plan of Correction: Per the provider: I will have the parents sign the acknowledgement for the records.
