Inspector Notes
An unannounced monitoring inspection was conducted on 5/22/24 from 11:41 a.m. to 1:49 p.m. with the Director. There were 52 children in care, ranging in age from four months to five-years-old, supervised by 11 staff. The children were observed preparing for naptime. Five child records and six staff records were reviewed. The center staff are current in certification for CPR and First Aid, and Daily Health Observation Training. Six medications and authorization forms were reviewed, and the center has six staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. Areas of non-compliance are identified in the Violation Notice. Violations
4Evidence: The TB screening on record for Staff #5, with an employment date of 5/13/24, was dated 3/4/24.
Based on observation, not all areas of the center were maintained in a clean, safe, and operable condition.
Evidence:
1. On the playground, there is an area where the artificial grass has several 2? round holes and grass was lifting posing a trip hazard.
2. There was a ? inch gap in the seam of the pour and play posing a trip hazard.
Evidence: In the toddler classroom, a child with a milk sensitivity was not included on the classroom allergy, sensitivity, and dietary restriction list.
Based on observation and interview; the center did not ensure soiled linens are stored in a leakproof covered receptacle.
Evidence: In the bathroom of the Montessori class, there was no lid and no liner for the laundry basket used to store soiled linens.
