During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 4/23/25 and completed on 4/23/25. The on-site inspection began at 9:00 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 51 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/12/25. A POC submitted after this date will not appear on the public website.

The required ratio for children ages three-years-old up to school-age eligible is 1 staff per 10 children.

Evidence: At 9:20 a.m. in the Prep classroom, 22 children were observed, ranging in age from three years to five years old, with two staff supervising. Staff stated that they had been over ratio by two children for five minutes.
Plan of Correction: Based on the record several children arrived at the same time, during circle time. The next staff was scheduled to arrive in at 9:15 a.m. and she had not arrived yet, 3 minutes later the staff arrived. Center director will continue to monitor trends and drop off time to adjust schedules accordingly.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:

In the Prep classroom, ages three to five-years-old, there were 22 children present, the written record reflected there were 21.

In the Two-year class there were 10 children present, the written record reflected there were nine.
Plan of Correction: The school uses two ways to track attendance, the digital attendance was correct, 5 mins later the child was added to the paper copy.

Center Director will maintain both written and digital record of attendance at all times.
