An unannounced investigation was initiated on 4/23/25 and completed on 4/30/25 in response to a complaint received by the licensing office on 4/12/25 relating to allegations of care, protection, and guidance, supervision, and injury reporting. The inspector reviewed one child record and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
