Inspector Notes

An unannounced monitoring inspection was initiated on 5/27/25 and completed on 6/10/25 in response to a self-report received by the licensing office on 5/15/25 relating to an injury requiring outside medical treatment. The inspector reviewed one child record and three staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center shall notify the parent by the end of the day for any known minor injuries.

Evidence: On 5/14/2025 while the Junior Kindergarten class, ages five-years-old, were on the playground, Child #1 had notified Staff #1 that they had injured their back while playing on the stairs of the composite play structure. Staff #1 reported that they had failed to write an injury report or notify administration of the injury before they left for the day.
Plan of Correction: Based on the record a Junior Kindergarten student notified staff about injuring their back on the playground stairs. Staff admitted to not reporting the incident or administration or written a report. All staff will notify administration of any injuries. The center will inform the parent of any known minor injuries by the end of the day.
