Inspector Notes
An unannounced investigation was initiated on 12/22/25 and completed on 12/22/25 with a local agencies in response to a complaint received by the licensing office on 12/15/25 relating to allegations of care, protection, and guidance. The inspector reviewed three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
