Inspector Notes

A monitoring inspection was initiated on 2/6/2023 and concluded on 2/6/2023. There were a total of 12 children, including 1 child of the provider's (31 points) observed with 4 caregivers present. The children were engaged in the following: outside play, staff were observed diapering, and handwashing. The home, 5 child records, 5 caregiver/household member records, evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of staff records, one staff does not have documentation of 2 references before employment.
Evidence: Staff #1, date of hire 1/23/2023 does not have documentation of 2 references. This staff was observed caring for children.
Plan of Correction: Provider will obtain 2 references for staff member.
Standard 8VAC20-800-60-B
Based on a review of children records, a child's record does not have documentation of medical policy number, written authorization for emergency medical care and policy for administration of medication.
Evidence: Child #1, date of first attendance 6/7/2022 does not have documentation of policy for administration of medication.
Child #2, date of first attendance 8/1/2022 does not have documentation of authorization for medical care.
Child #3, date of first attendance 8/17/2022 does not have documentation of medical insurance policy number.
Plan of Correction: Provider will ask parents to complete the missing information.
Standard 8VAC20-800-830-A
Based on a review of emergency drills, the emergency evacuation procedures have not been practiced monthly with all caregivers and children in care.
Evidence: There is no documentation that fire evacuation drills were conducted for the months of September 2022, October 2022, November 2022, December 2022 and January 2023.
Plan of Correction: Fire drills will be conducted monthly and documented on the fire drill form.

Based on a review of emergency drills, there was documentation that only 1 of the 2 shelter in place drill was conducted per year.
Evidence: Only one shelter in place drill was conducted in the year of 2022.
Plan of Correction: 2 shelter in place drills will be conducted during the 2023 calendar year.
