Inspector Notes
A monitoring inspection was initiated on 8/7/2023 and concluded on 8/7/2023. There were a total of 12 children and providers own child ( 33 points) observed with 4 caregivers present. The children were engaged in the following: music and movement, banana number game, coloring monkeys, diapering, bathroom and handwashing. The home, 4 child records, 4 caregiver/household member records, evacuation drills, emergency supplies and attendance were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 1Based on a review of children records and provider interview,children's records do not have written authorization for emergency medical care, policy for administration of medication and documentation of physical and immunization record.

Child #3, date of first attendance 5/22/2023 does not have documentation of child's physical and immunization record within 30 days of the child's fist day of attendance.
