Inspector Notes
An unannounced renewal inspection was conducted on January 19, 2024, from 9:45am-10:45. At the time of entrance, ( 11 ) children ( 25 ) points were in care with ( 3 ) caregivers present. The inspector reviewed 4 children?s records and 3 caregiver/household member records. Children were engaged in group activities, coloring the letter U, math with cotton balls and Mr Potato head. No medications were on site.
Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

1Based on a review of children records, 2 children records do not have all the required information to include, 2 emergency contact information, proof of identity, written authorization for emergency care, documentation of child's physical and immunization records.
Evidence: Child #1, date of first attendance 2/21/2023 does not have documentation of a second name, address and phone number of a person to contact in an emergency, no proof or identity and no written authorization for emergency care.
Child #2 (repeat), first day of attendance 5/22/2023 does not have documentation of child's physical and immunization record within 30 days of the child's first day of attendance.
