Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 10/28/2021, the virtual on-site inspection was concluded on 11/04/2021 and the inspection concluded with a record review on 11/10/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 20 children, ages 3 years-5 years, and 7 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 3 children?s records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to ensure that a person must not be employed until the agency has the person completed sworn statement or affirmation.

Evidence: The record for Staff 1 had a documented hire date of 08/09/2021; the date on the sworn statement was 08/10/2021.
Plan of Correction: The sworn statement will be a part of the application process.

Based on record review and discussion with staff, the center failed to obtain findings from a search of the central registry within 30 days of employment.

Evidence: The record for Staff 1 had a documented hire date of 08/09/2021; there were no central registry findings available for review during the 11/10/2021 record review inspection.
Plan of Correction: Will be completed within 30 days.

Based on record review and discussion with staff, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.

Evidence: The record for Staff 1 had a documented hire date of 08/09/2021; the date on the employment eligibility letter was 08/13/2021.
